A buddy of mine had this issue once. Hooked an electric vacuum pump up to
the bleeder valve and everything. I took a peek at the master cylinder and
noticed he'd put the black rubber diaphragm, that normally rides on top of
the fluid, in and then poured the brake fluid on top of that.  Sounds like
an obvious thing to check, but just thought I'd share.
