Thanks for the issue @ronag. This is a good thing to fix. It's been an issue
we've encountered over the years and have been meaning to fix it just never go
to it.
A `heartbeat_seq=true` parameter to `_changes` feed that returns heartbeats
`{ seq, changes: [], id: null }` could work. We could teach the replicator to
pass that parameter (it will be ignored on older versions) and then expect that
shape of heartbeats to come in and checkpoint accordingly.
