get a single one on timeout?

Without a `heartbeat` parameter and with a `timeout` we'd use the timeout as
the spacing between seq heartbeats. By default the timeout is 60s. So if the db
is busy going through changes and drop them all, we'd get about 1 every minute
(replacing the newline heartbeats). Then when reach the end if nothing is being
written the timeout would trigger and we'd get a `last_seq` just like today.

If we have a `heartbeat=N` set, the feed stays open and `N` is the spacing
between seq heartbeats.
