this issue still exist in ctakes 4.0.0
a workaround I would suggest - edit sno_rx_16ab.xml found in
C:\apache-ctakes-4.0.0\resources\org\apache\ctakes\dictionary\lookup\fast
Modify and save the 4 "CHANGE_ME" parameters in the file to your UMLS user and
password.
