Continues #15801. Splits out the part jdaugherty already agreed on: file renders default to `Content-Disposition: attachment`, unsafe filenames get escaped, and `inline: true` opts out. Leaves the `text/plain` default for inspect/unrecognized map renders for the weekly discussion.
