The guide now states the pessimistic default that you verified. `rejectIfNoRule` defaults to true, so a URL with no annotation, `controllerAnnotations.staticRules` entry, `interceptUrlMap` entry, or `Requestmap` is denied. The `@Secured` section says an action with no access rule is denied unless strict mode is turned off with `rejectIfNoRule`.
