The telco/isp hands you an Ethernet drop using some sort of onsite equipment. 
What they use to deliver it is up to them. This is no different than ordering a 
T1 except rather than needing a T1 card in your router, you need an Ethernet 
port.

They don't have to use the MPLS - that's just an option.  They can use any kind 
of platform - ATM, FR, etc.  Whatever the costs drive them to use.

How is it simplified from MPLS if they have to have the MPLS in the first place?

I think it is MPLS but by hosting services rather than phone carriers, who 
would use the phone carrier's MPLS circuits as the backbone.

Well, by extension, that seems rather simple.  It's a four letter word that 
begins with t, followed by h, that ends with at.  I find that very often things 
are explained by where you is at.
