Maybe I'm behind too - sorry. Anyway, we have a homegrown app that uses .net and asp code to dish up a website that does exactly what you described. User's AD login is pullled and their name, etc are populated into the form. Then they fill in the checkboxes for type of server Prod, Pre-Prod, etc., OS if not standard - eg 2003 currently for us, VM or Physical (defaults are all VM unless they have a good reason), any customizations outside our standard # of cpu, memory, and drive configurations. Then they save it, the system assigns a number, sends them an email confirmation, and also forwards on the "order" to our build teams. Once it's built, the engineer goes in and updates the status which sends out email to remind the user to request backup & monitoring if applicable.

Currently, someone just emails the system admins what they need and we take it from there.
