I've started work on my own campaign world... in 3E/ d20 system format   
Creating new monsters has been a real big pain in the arse... but I usually 
model the creature I'm trying to 'build' off of a creature already described 
in the Monster Manual... adn then go from there.  I also run groups of 4 PC's 
of varying levels (i.e. 4 1st lvl, 4 2nd lvl, etc.) to test the CR's of 
different creatures.
