It USED to live at:

. . . but disappeared from there sometime AFTER September 30, 2000.  I 
cannot now find it ANYWHERE.  Perhaps it has been "rescinded".  If ANYBODY 
finds it let me know, as there are a LOT of people I know that really want 
to review the specifics again.
