John,

Here's my take.  I read the d20STL, d20 guide, and OGL to allow the following:

1.  d20 Guide Restricted TM Use section:  "...you may not use the Dungeons & Dragons or Wizards of the Coast trademarks in advertising or in any marketing in support of the Covered Product, ...."  This language refers to those specific trademarks, not all Wizards and DnD trademarks.  Secondary support for this interpretation comes from the specific mention of the Dungeons & Dragons and Wizards of the Coast trademarks on page 3 of the guide, and paragraph 1 of the d20 STL which again makes reference only to those trademarks in the d20 guide.

However, because Ryan has made it explicitly clear that Dungeon Master's Guide is a registered TM, but that he thinks the use of "DMG" is o.k., I would simply suggest something along the lines of:  "...by Monte Cook, author of the Third Edition DMG!"  This is of course b/c normal I.P. law is still in effect.

2.  As to including that info in the "About the Author" section, I see no problem.  It's a fact that M.C. wrote the DMG.

(2) Am I violating the OGL or D20STL if I mention, in advertising for
Monte's adventure, that he wrote the Dungeon Master's Guide (which includes
a WotC trademark)?  Or if I include that info in an "About the Author"
biography in the book?
