Can someone identify this problem for a book that is /not/ part of the
lending library, but is unencrypted and can be downloaded by everyone
without sitting on a waiting list?

At first I thought I'd just found one or two books where this
happened, but I've run into it often enough that I now think there's a
systematic problem where epub versions of many books are completely
missing the first page or two of the text. For example:
https://openlibrary.org/books/OL10684039M/Farthest_Star
https://openlibrary.org/books/OL9214083M/The_Wild_(Requiem_for_Homo_Sapiens)
https://openlibrary.org/books/OL22973007M/Past_times
The PDF versions are OK (if huge), but the epubs are (according to
ADE, anyway) missing the first page or two in the 'Pages' section.
Can anyone else verify this? Any ideas what's wrong? Is ADE simply
skipping over those pages in the epub for some reason, or are they
actually not present in the document? Maybe it resulted from a
systematic problem with scanning during a certain time period?
