I am assuming that u r using MPCOS cards and Kit by gemplus. Hope u have got
manuals along with it.
If u go thro them, u can find details about doing personalization.

Personalization is nothing but creating required files and writing
cardholder+ application specific data into files.
If it is testing and learning purpose u can try with Pilot( I am not very
sure ).

Once u set the personalization bit u can not do anything....creating
files/erasing etc..... But u can read/write/update if u have given
permissions while creating those files.

THe answer to second question is,

    For any elementary file u can give read+write+update permissions. For
secret code file, after personalization either lock/localize the access
conditions( permissions) so that nobody can change the secret codes.
