I found the problem. You will need one more image definition for the Server Type. XML fragment is below for this definition.

            icon="icons/obj16/openejb.gif"
            typeIds="org.apache.openejb.eclipse.wtp.server"/>

Also, it would be nice if we rename the image folder to icons ( :-) just trying to follow the Eclipse conventions) and create a sub folder named obj16 to hold 16x16 images. I made this change in my working copy. Should I attach the patch here as we do not have JIRA issue yet.
