At UMHS the medical school operates and staff's many outpatient facilities that are in the community. However, all of these use the systems provided by and supported by the Hospital's professional IT staff.
This is what I describe as one kind of niche. This is the niche that VISTA is clearly adapted for and because that market is much smaller than primary care operations (at least world wide, it would be interesting to know the mix in the US) that are small group practices, I believe that VISTA stands the greatest chance of all OSS efforts in that niche to be successful.
It's the other niche's that I feel the situation is less than clear.
