I think the idea is not to use an external reader but the MicroSD card with a smart card inside. I found [1] and [2]. Talking to the smart card uses the MicroSD interface and may look like a hack. I don't know if the "Mobile Commerce Extension protocol (standardized by the SDA)" or MCEx protocol is public. I do not have such a MicroSD myself.
