Either per-session or per-user (i.e. a cookie is used to determine what set of URL's has been associated with actions).

Even that seems odd.Where is the "token" or "key" that
provides the handshaking that is required to tackle this problem. And to
clarify again, the problem is: "If the submit button is pressed twice on a
form, I want the action only to execute once".
And that is exactly what happens.
