Sounds like a bug... please file a jira issue (and provide a patch if
you can!)

If I use two action tags on same page and the action-name and namespace 
are equal on the two separate tags, the second invocation is not 
creating a new Action object. This is not the supposed behaviour, or is
it?

In my case I was setting some tag params and they were set in the second

invocation too, even if I didn't include them in the tag params. (Thus 
my conclusion that the object is reused; also, the log shows no param 
setting in action)
