Ok, I guess you can try this to see if you can get your device fixed. mkfs -t msdos -c partition-name
where partition-name is the name of the disk partition on which you wish to put the file system, usually the beginning of the disk. This will make an ms-dos type file system, onto which you can place the windows installation. The -c option will check the device for bad blocks and, prayerfully, not use them for creating the file system.
