On what planet? Certainly nowhere on earth. People who provide medical  
goods and services in the competitive market in the US are called drug  
dealers and  quacks.

In economic terms the government is just an enterprise that  
specializes in coercion to generate a revenue stream. It does have  
some competition. Private security firms compete with the police and  
private armies compete with the military. Certain forms off crime also  
compete with government. In some cases, the IRA, the Taliban, the  
Mafia in some communities, organized crime actually becomes the  
primary governmental body.
