That looks correct. I don't know why you are getting this error. There is one thing you could try that may help: download the official OTB 5.6.1 windows package from https://www.orfeo-toolbox.org/download/

As far as a fresh reinstall, I mean that I went through OSGEO4W setup and "uninstall" every library and package, deleted all files from my computer associated with osgeo4w and qgis/otb/grass, and then reinstalled everything using osgeo4w setup, to find the same issue. I can run OTB standalone through monteverdi, but running it as a qgis plugin is greatly preferred.
