That looks correct. I don't know why you are getting this error. There is one 
thing you could try that may help:
download the official OTB 5.6.1 windows package from 
https://www.orfeo-toolbox.org/download/

then unzip it somewhere and update the config paths to point to this 
installation of OTB:
OTB applications folder should point to OTB-5.6.1/lib/otb/applications
OTB command line tools folder should point to OTB-5.6.1/bin

In my version of qgis that menu is processing -> Options -> Providers -> OTB, 
but I do have my config paths set as shown, which is the recommended setting 
according to the the guides I have been able to find

Yes I am running from QGIS 2.16.3.  As far as a fresh reinstall, I mean that I 
went through OSGEO4W setup and "uninstall" every library and package, deleted 
all files from my computer associated with osgeo4w and qgis/otb/grass, and then 
reinstalled everything using osgeo4w setup, to find the same issue.  I can run 
OTB standalone through monteverdi, but running it as a qgis plugin is greatly 
preferred.

Have been using OTB for a couple weeks now, but went in to osgeo4w today to add 
the GRASS plugin and after doing this my OTB tools no longer work.  Whenever I 
use any of them I get the error "otbapplicationlaunchercommandline.exe has 
stopped working".  In the log, the only error is

"The following layers were not correctly generated.

  *   Output vector file

You can check the log messages to find more information about the execution of 
the algorithm"


I've now tried a fresh reinstall and have the same errors.  Has anyone gotten 
this issue before or have any ideas to solve it?
