While in the OTRS GUI you can't modify articles, the API does support this. So depending on your needs; it would be pretty simple to write a command line script that goes into an article, exports the attachments and puts in a message like "content removed because of confidentiality by <name> on <date>". This can even be included in the GUI as an action, "Forward", "Bounce", "Extract"...
