I know that birth plans are given to expectant families in antenatal 
classes, but we don't often see them on arrival to the hospital.  And the 
main women filling them in are first time Mum's.

I find the Mums second or third (or more) time around are more verbally 
assertive, and can tell you (and the doctor) in no uncertain what is NOT 
going to be done.  At least they know what they want??
