I've experienced that a lot. Sometimes running a full text crawl rectifies it, but not always. I've found the easiest way to sort it out is to disable any workflows that run when the form is modified, create a dummy column, open the library in datasheet view, and then fill the dummy column with a 1 or something. That forces it to refresh somehow.
