In the airline industry, when a plane goes down, investigators dive in deep to the various causes. They establish the "accident chain" because there is never any one cause. We fixed all the "one causes" about they time they invented flaps and rudders. The mission is to learn to do better; there is no interest whatsoever in ascribing blame to the people at Skype.
