I forgot to update this thread with the cause and solution... The problem was caused by our app deleting a database chunk while it was still owned by the database. This would only happen under rare circumstances so we missed it and it was not caught by the gremlin testing we performed. Probably would have been caught by gremlin testing eventually - statistically it should be. So if you have a similar symptom, you might want to check for this type of bug. Solution was a hard reset of the user's hh.
