Colin,
What comes to mind is that it could be the G4 upgrade. I believe that I've read
this a couple of times in the past.
Solution was to take out the upgrade, install the OS, than put it back in and
install the needed inits for your G4 upgrade.

Also you say:

Usually the disk drivers will be updated after that check. Did you format and
partition your HD with Apple's Drive Setup or a third-party app? If third-party,
it could be that that is your problem. If so, try turning off 'Update disk
drivers' and update the HD with an updated version of that third-party app.
