Since August, the word "insurance" appears in the Swap List almost 400 times but always in context, i.e., "...if you want insurance send...." In the digest this might mean the word pops up as many as a dozen times in one issue. It's possible if a filter looks for multiple occurrences of specific words that this would be above their threshold.
In that same time period, "insurance" shows up about a half-dozen times in each of the other LEM digests but at the most three times in a single issue and always in context..."install a UPS just for added insurance" etc.
If your ISP claims "ads" are showing up, that would imply more than text. LEM digests don't allow html code or attachments. Conclusion: they're blowing smoke.
