OWC.  I've seen a lot of inflated prices for various versions of Mac OS, but 
OWC has been reasonable.

Also, Note that you may be able to remake the install disk by copying all of 
its files over to the hard disk(drag the entire disk, so it gets copied as a 
folder).  Then you burn the contents of that folder to disk, but adding a 
"Desktop Folder" to it before you burn it, and it should, technically, come up 
as a startup disk, and function like the old one.  I've done this, and it has 
worked, but it is a little flakey.  This worked, when I tried to do an install 
over the network, or some version of Mac OS, but couldn't, and then...after a 
couple years passed, I had lost/misplaced that original CD, I was able to 
reconstitute it...and install on a 7200.
