Acrobat WebCapture converts html to pdf, and ATTEMPTS (often with a great
degree of success) to TRANSLATE form fields along the way into PDF fields.
Form field names are often a casualty in passing, but it is relatively easy
to apply renames as needed with the Forms palette.
