I can't think why the form fields would spontaneously go blank after sending the form to print. However, the users will not be able to save their form field data unless they have full Acrobat (5) or Acrobat 6 Pro. Acrobat/Adobe Reader will not save data inserted into by a user into form fields. Are you saying that after you have typed in values into the form fields, you print it but the form fields are blank on the paper when it prints out? I've no idea why this is happening.
