Approval is still available (in bulk; not as a consumer/end user product).

As stated before, the price of the comparable licencing model for Adobe Document Server for Reader Extensions is kind of comparable too (with the exception that you get more possibilities with the latter ... remember the word "with Approval, you can not approve" ... because Approval does not support commenting...).

To my knowledge, Approval has been discontinued as a product that Adobe is offering, they are pushing their Reader with Extensions (much more $$$$ for them). So we now have one less option to work with. It all really comes down to who will be using the forms that you want people to be able to edit, internally (within your own company) or externally (anyone else who has Acrobat Reader 5 or 6 and needs to use your forms).
 
You then get into the area of who authors the form and how you want the form to be used within browser or externally.
