Please keep in mind - to quote Max Wyss - Reader is a
"READER" not a "SAVER". Someone will have to incur a cost to allow the end-user
to SAVE form data.

You cannot "save" a PDF form with Reader, without the extensions embedded in your PDF. And it is expensive.
