When I've seen this in the past, it was because the first-level bookmark
wasn't in the same text flow as the subordinate bookmarks. I'm guessing that
you have a separate flow set up for your chapter title (or whatever the
first-level bookmark is). If you put the chapter title in the same flow as
the rest of the file, I think this issue will go away.

This is my first intrusion on this list. I'm having a shifted bookmark
problem in PDF files generated from FrameMaker 6.0-Acrobat Distiller 4.05
from a book and files. My use of Acrobat and Distiller is pretty simple and
straightforward, since I simply generate PDF from FrameMaker files.

The second level bookmarks are displayed in the first level bookmarks
preceeding the one where they should go:

Chapter 1 First level bookmark
      Chapter 1 Second level bookmark 1
      Chapter 1 Second level bookmark 2
      Chapter 2 Second level bookmark 1
      Chapter 2 Second level bookmark 2
Chapter 2 First level bookmark
      Chapter 3 Second level bookmark 1
      Chapter 3 Second level bookmark 2
