This can be done in Acrobat 6 through a batch process (see Acrobat Help for how to run a batch process... and if that is not enough you might also have a look on PlanetPDF for some material by D.P. Story about that.)
In that batch process you execute a JavaScript which uses the setAction() method, which adds a script to the specified action (see Acrobat JavaScript Scripting Reference for more about that method).
Then run the Batch, and there you are...
