You are right that many programs seem to manage this case better than I describe. I suspect most programs, the great majority, use platform text layout tools to do their job - GDI in Windows, QuickDraw in Mac. So you should probably consider either using these tools, or discovering their behaviour and emulating them. Bear in mind that there is nothing specifically "right", because none of these programs do anything like good typography.
