I almost forgot: This book also includes a CD with a large selection
of legal documents for you to use: Model releases, property releases,
art and merchandise licenses, and a whole lot more.

Another one is "Business and Legal Forms for Photographers" by Tad Crawford (Allworth Press, ISBN 1-880559-82-X); also with CD.

The author blurb on the back says:
