Have you ever seen the Lester Dine 105mm lenses and ring flashes?
Those were specifically designed for dental photography, and they
aren't small by any means.

Lester Dine actually created (or had them made for them) ring flashes
for dental work, way back in 1952.  Sunpak rebadged some of their
ringflashes for Lester Dine.
