If you were looking for one simple BOM format, you are out of luck. There are several IPC documents that may be relevant. You have to pick through them to find out what applies to your situation. Each of the documents mentioned below will also give references to other documents that might apply.

IPC D-325A - Documentation Requirements for Printed Boards (describes the parts list)
This document has to be purchased from IPC, and can be found on the IPC website in the Standards section


IPC 2578 - Sectional Requirements for Supply Chain Communication of Bill of Material and Product Design Configuration Data - Product Data eXchange (PDX)
This can be downloaded free at - http://webstds.ipc.org/2578/2578pub.pdf


IPC 2571 - Generic Requirements for Electronics Manufacturing Supply Chain Communication - Product Data eXchange (PDX)
