Yes, AJ. It's all about tolerances. If they drill the top layer with a little offset to the left and the inner layer is shifted to the right and you have small clearance between the polygons and the holes (with no pads), you may get the top pad shorted to the inner copper when the hole is plated. No need to mention that the fab never drills only one board. They pick 3-8 depends on the thickness and the drill bits. The hole on the top is never the same size as the hole on the bottom of the so formed package. I think you got the picture.
