I think that "somewhere" there is a "Fanout" command provided with versions
following Protel 99 SE, which can be used to route connections between
surface mount pads and Internal Plane layers, but I'm not sure offhand if
such a command is also provided with Protel 99 SE. (Check out the
functionality provided by the autorouter?)

In the event that such a command is not provided though, you would need to
route such connections yourself. To connect each SM pad to an Internal Plane
layer, you would need to add a track (on the same layer as each pad
concerned) and a via (with the via located at one end of that track and the
pad at its other end).

Sorry if that sounds obvious, but that track and via *are* required, and
regardless of whether you provide them yourself or whether they are
otherwise provided by some command. (And unlike in versions before Protel 99
SE, vias *can* connect to Internal Plane layers; it is no longer necessary
to use through-hole pads instead to connect to those layers.)
