Hi Everyone, 

I would greatly appreciate and love to hear any thoughts on the following... 

I have been testing for pituitary disorders like crazy. Here is what I know so 
far: 

microadenoma on pituitary, cyst on pineal gland (confirmed by two MRIs) 
multiple cysts on thyroid (ultrasound) 
hypogonadism (low estrogen, lutenizing hormone, amenorrhea, breast loss) 
hypothyroidism (low T4) 
intermittent high cortisol (urine, saliva, never high in blood) 
cortisol did not stimulate on insulin tolerance test, but was not high in blood 
at baseline (8 am.) 
low white blood cells 
high albumin/globulin ration 
low globulin 
heart murmur 
various dermatologic diagnoses - keratosis pilaris, cystic acne, telugen 
effluvium, seborrheic dermatitis 

I have a slew of other symtoms, but none of the "classic", superficial 
Acromegaly or Cushing's symtoms - no hand/foot/nose growth, teeth spacing, 
central obesity, buffalo hump... 

Here are my recent OGTT and IGF-1 results: 

7:56 am. 
Glucose 87 mg/dL 
GH 5.2 ng/mL 

9:08 am. 
Glucose 139 
GH 6 

10 am. 
Glucose 89 
GH 6.4 

11:13 am. 
Glucose 46 
GH 5.6 

Glucose Interpretation: 
7:56 am. - normal <100 (65-99 mg/dL) 
9:08 am. - diabetes >=200 
10 am. - impaired tolerance 140-199; diabetes >=200 

There are no reference ranges given for the 11:13 am. glucose result, but the 
report marked it L for low. I THINK that indicates an impaired pituary response 
(shocker). 

Okay, so now for the interesting/complicated stuff... 

GH Interpretation: 

Because of a pulsatile secretion pattern, random (unstimulated) GH levels are 
frequently undectectable in normal children and adults and are not reliable for 
the diagnosis of GH deficiency. Regarding suppression test, failure to suppress 
GH is diagnostic of acromegaly. 

Typical (GH) reponse in healthy subjects: 

Using the glucose tolerance (GH Suppression) test, acromegaly would be ruled 
out if GH level is <1.0 ng/mL. 

Using the GH stimulation test, the following results would rule out GH 
deficiency: 

Adults (> or = 20 Years): 
Insulin hypoglycemia > or = 5.1 ng/mL 

Insulin Like GF (IGF-1)

5/7/2008 7:59 AM 323 138-410 ng/mL
