bugfix/RFE:

This does not implement everything that was discussed in Dec. 2011-Feb.
2012 [1].
However, it is arguably far better than what we currently have and is
actually relatively little new code (if you exclude unit tests and
comments it's ~350 loc).
I'll likely add more unit tests, but the implementation should largely
be clean and close to final at this point. Just want to get this out
there to peek and poke at.
