Hmmm... i think you should consider squid's proxy authentication. Squid supports a number of authentication methods... LDAP NCSA SMB PAM etc..
once the users have authenticated then you can use "authentication on access contol" (http://www.squid-cache.org/Doc/FAQ/FAQ-23.html#ss23.4) CAVEATS: This might require some initial work depending on the authentication method you'd be using. I think if you have an existing SMB/LDAP server, you might want to use those to minimize the work to be done.
