What is the failing behavior?  Does anything show up on the screen, any
messages?

Fwiw, I had trouble with a particular brand of USB drive (the no-name
bamboo-cased $5 8GB drives available from MultCoLib), but switching to
random swag from OSCON worked.  Even in the failing case, I got some
messages though.  I dd'd the image to the usb drive's block device (CHECK
CAREFULLY).
