I have gigabit through CenturyLink. You can get a fixed IP address. Costs
$5/month I think. You have to ask and if they say they can't, talk to
someone else.

Setup was pretty easy. Figure out what you want for your internal subnet
and configure it into the router. IPv6 is supported. DHCP is provided by
the router.
