At Jive we are currently using a combination of monitoring solutions. We've used OpenNMS for years and still use that. I'm assuming you've already looked at this and it falls into your "great for tier 1 but not 100% reliable" category. We've recently (last 6-12 months) started using NewRelic and Boundary and have been pretty pleased with them, but perhaps you looked at these also and filed them into your "blatant rip off" category. We think they are a pretty good value, but I'm afraid that is probably a subjective assessment.
Where are the open source monitoring solutions falling short? If it is that they aren't comprehensive enough, I would suggest taking a look at Ganglia if you haven't already. It is pretty easy to get Ganglia to monitor pretty much whatever you want to monitor by writing your own extension. Maybe, however, you are simply wanting to offload this responsibility to someone else and not have to worry about it anymore.
