Try to enable "info" level debugging on "plugin.customscan" in SBS Settings/Advanced/Logging and see if it seems to hang on a certain query in the "refresh" operation that happens as the first step of a rescan. With "info" level logging you will be able to see how long time each step in the refresh operation takes in the server.log.
I solved some issues in one of the latest Custom Scan versions related to this but I've seen that it can take some time in my own fairly small library, so I suspect it might take longer in a large library.
