It doesn't seem to be any problem with your Custom Scan configuration.
Can you temporarily install the "Database Query" plugin and post the
statistics from the "Custom Scan Statistics" and "Squeezebox Server
Statistics" reports provided through it ?
Also, do you get duplicates for all items or just some of them ?
