did you make test using your map file and * mapserv cgi * shp2img * a simple PHP Mapscript script, like test_mapfile.phtml (at least included in pmapper 1) ????
This mailing list is mainly for p.mapper related questions, so one should first try to sort out if any problem might be related to errors in basic configuration (map file, web server, file permissions, etc). If the image cannot be created then you will probably have missing write permissions for the user account running your web server, or the tmp dir is not existing, whatsoever.
